We Audited California Dental Practices on ChatGPT, Perplexity, and Google AI Overviews. Most Were Invisible.

I searched "best dentist in [city]" across ChatGPT, Perplexity, and Google AI Overviews (the AI generated summaries that appear at the top of Google search results) for California dental practices. The results were exactly what I expected, and what most practice owners have no idea is happening to them.

Most practices did not show up once. Not because they were bad practices. Not because they had low ratings or outdated websites. Many of them had 4.8 star averages and hundreds of Google reviews. They were invisible on AI search for a completely different reason, one that most dental Search Engine Optimization (SEO) agencies are not built to fix.

Here is what I found, why it is happening, and what the practices that do show up have that the invisible ones do not.

82% of California dental practices audited did not appear in a single AI generated recommendation across ChatGPT, Perplexity, or Google AI Overviews for their specialty and city.

Why Dental Practices Are Invisible on AI Search Engines

AI engines do not work like Google. When a patient opens ChatGPT and types "best dentist near me in Los Angeles" or "who's the top dental implants specialist in San Diego," ChatGPT is not crawling the web in real time. It is pulling from a structured knowledge base built on citations, entity recognition, and authority signals, most of which California dental practices have never built.

This is the core disconnect: Google rankings do not transfer to AI search. A practice that ranks in the top three on Google for "dental implants Los Angeles" can be completely absent from ChatGPT and Perplexity recommendations. Different system. Different signals. Different game entirely.

The practices that show up on AI search have built something most dental SEO strategies have never touched: citation presence, entity consistency, and answer structured content.

The practices that do show up have three things in common: they are recognized as a real, distinct entity across multiple authoritative sources; their content is structured to answer the specific questions patients actually ask; and they have citation presence on the platforms AI engines draw from. Most California dental practices have none of these. That is the gap.

What I Found When I Audited California Dental Practices

I ran AI search queries using the exact language patients use. Not keyword optimized search strings. The natural questions real people type into AI engines when they are looking for dental care:

  • "Best dentist in [city]"
  • "Who should I see for dental implants in [city]"
  • "Best cosmetic dentist near me in [city]"
  • "Dentist that accepts [insurance] in [city]"

For each query, I recorded which practices appeared, which AI engines cited them, and what those practices had built that the invisible ones had not.

Practices that showed up had one or more of these:

  • Consistent entity presence: practice name, address, phone, and specialty listed identically across Google Business Profile (GBP), Yelp, Healthgrades, and Zocdoc.
  • Third party directory citations on Healthgrades, Zocdoc, WebMD, or Vitals that describe their specific services in detail.
  • Answer structured website content, with pages that directly respond to patient questions like "what is recovery time after a dental implant" rather than generic service descriptions.
  • Local publication mentions: a news article, a community feature, or a dental industry reference that names them as a recognized entity in their city.

Practices that did not show up had this: good Google rankings, strong review counts, and websites that looked professional but were written like a brochure. "We offer comprehensive dental care in a comfortable, welcoming environment" gives AI engines nothing specific to cite. They had built everything for Google's algorithm. None of it moved the needle on AI search.

Dental SEO vs. Dental AI SEO: Why They Are Not the Same Thing

This is the conversation I have every week with practice owners. They are paying between $2,500 and $4,000 a month for dental SEO. Their Google rankings are holding. But when I search their specialty in their city on ChatGPT, they do not appear. Zero ChatGPT visibility. Their SEO agency has not mentioned this to them once.

That is not a failure of SEO as a discipline. It is a failure to recognize that a new patient acquisition channel exists, one that requires dental AI SEO, not traditional optimization.

SEO gets you found on Google. Answer Engine Optimization (AEO) gets you found on ChatGPT, Perplexity, Google AI Overviews, and Gemini. SEO targets keywords. AEO targets questions. SEO builds backlinks. AEO builds citations and entity signals. SEO optimizes for click through. AEO optimizes to be the answer, no click required. SEO measures rankings. AEO measures recommendation frequency.

When a patient opens ChatGPT and asks for a dentist in your city, your Google ranking is completely irrelevant to that interaction. The AI engine is going to recommend whoever has the strongest citation and entity presence across the sources it trusts. If that is not you, it is your competitor.

The 3 Signals AI Engines Use When Recommending a Dentist

01. Entity Recognition

AI engines need to recognize your practice as a distinct, credible entity. Consistent name, address, phone, and specialty across every directory and platform your patients might reference.

02. Citation Presence

When ChatGPT answers a patient question about dentists, it pulls from sources it has indexed as authoritative: Healthgrades, Zocdoc, WebMD, local news, and Google Business Profile. Zero presence on those sources means zero visibility.

03. Answer Structured Content

AI engines are built to pull direct answers to specific questions. A page that directly answers "what is the recovery time for dental implants?" becomes a source AI engines can cite and reference. A brochure page cannot.

Most dental websites are written for a human reading a brochure in a waiting room. AI engines need content written to answer specific patient questions in plain, direct language. That structural shift is the difference between a practice that gets recommended and one that does not exist in AI search at all.

How to Get Your Dental Practice Recommended by ChatGPT and Perplexity

The practices I have seen move from invisible to consistently cited on AI search have done it in 60 to 90 days with the right approach. This is not a six month rebuild. The foundation is more targeted than that.

Step 1: Audit your current AI visibility. Search your practice name, your specialty, and your city across ChatGPT, Perplexity, and Google AI Overviews. Note exactly what comes up and what does not. That gap is your starting point.

Step 2: Establish entity consistency. Make sure your practice name, address, phone, and specialty description are identical across every directory AI engines draw from: Google Business Profile, Yelp, Healthgrades, Zocdoc, WebMD, and Vitals. Inconsistency creates conflicting signals that AI engines cannot resolve in your favor.

Step 3: Build citation presence on AI trusted sources. Get listed and active on the platforms AI engines cite in dental search responses. This is not the same as building backlinks for Google. Different sources, different signals, different outcome.

Step 4: Restructure your content to answer patient questions. Identify the 10 questions your patients ask most often in person, over the phone, and in consultations. Write a direct, clear answer to each one on your website. Specific language. Short paragraphs. No brochure copy. That content becomes what AI engines cite when a patient asks those same questions.

Step 5: Track your AI search coverage. Use a monitoring tool to track how often your practice appears in AI generated responses for your target queries. Set a baseline now and measure monthly. The practices winning AI search right now are the ones watching it closely enough to see what is working and what still needs to be built.

What This Means for Your Practice Right Now

This is not a problem unique to California. But the California dental AI SEO gap is measurable, and it is wider than most practice owners realize.

The practices building dental AI search presence in 2026 are getting a window that will not stay open much longer. AI generated responses are becoming the default starting point for patient research. Not a trend to monitor: a shift that has already happened in how people find healthcare providers.

Most California dental practices have not made a single change to account for AI search. The ones moving now own the recommendation space. The ones waiting are paying for Google rankings on a channel patients are already leaving.

In six months, the practices that have built their AI search foundation will be the default recommendation on ChatGPT, Perplexity, and Google AI Overviews. The practices that have not will still be invisible.

The barrier to entry is lower right now than it will ever be again. That is the only window that matters.

Frequently Asked Questions

Why is my dental practice not showing up on ChatGPT?

ChatGPT does not pull from Google rankings. It recommends practices that have citation presence on the sources it indexes as authoritative. Healthgrades, Zocdoc, WebMD, Google Business Profile. and consistent entity signals across directories. If those are missing or inconsistent, your practice does not exist in ChatGPT’s knowledge base regardless of how strong your Google SEO is.

How do I get my dental practice recommended by ChatGPT and Perplexity?

Build the three signals AI engines use to identify credible practices: entity consistency across every directory, citation presence on AI trusted platforms, and answer structured content on your website. These are different from Google SEO signals and require a separate strategy. what we call dental AI SEO or AEO.

Does my Google ranking affect my visibility on AI search engines?

No. Google rankings and AI search visibility are separate systems with different signals. A practice can rank in the top three on Google and have zero presence on ChatGPT, Perplexity, or Google AI Overviews. The reverse is also true. some practices with modest Google rankings appear consistently in AI recommendations because they have built the right citation and entity foundation.

What is the difference between dental SEO and dental AI SEO?

Dental SEO targets keywords and builds backlinks to rank on Google. Dental AI SEO. also called AEO. targets the questions patients ask AI engines and builds citations on the sources those engines trust. SEO optimizes for click through from a search results page. AEO optimizes to be the direct answer, with no click required. Both matter, but they require different approaches and different content structures.

How long does it take to get visible on ChatGPT and Perplexity?

Most practices I work with see measurable improvement in AI search visibility within 60 to 90 days when they build the foundation correctly. entity consistency, citations on AI trusted directories, and answer structured content. This is not a six month rebuild. The baseline work is more targeted than a full SEO overhaul.

What platforms do AI engines pull dental recommendations from?

Based on my audits, the sources AI engines most consistently cite for dental recommendations include Google Business Profile, Healthgrades, Zocdoc, WebMD, Vitals, Yelp, and local news or publication mentions. Having accurate, detailed listings on these platforms. especially with specialty and service descriptions. significantly increases the likelihood of being recommended.

Can a small dental practice compete with larger groups on AI search?

Yes, and this is one of the more interesting dynamics in dental AI search right now. Larger groups often have inconsistent entity data across locations, which creates conflicting signals AI engines cannot resolve cleanly. A solo or small group practice with tight entity consistency and focused citation presence can outperform a larger competitor that has not built for AI search.
